We provide a range of services for older people in our communities, based on assessed eligible need. This could include support in your home and daily life, following a spell in hospital, or in a care home.
Homeless Health
The Homeless Health Team provide a flexible outreach service to people who have concerns about their mental health and are homeless or at risk of homelessness in West Lothian.
